Medication at School
The SUHSD complies with and is governed by state law regarding medication on campus. Education Code 49423 states that any pupil who is required to take medication during the school day may be assisted by the school nurse or other designated school personnel if:
- The school receives a written statement from a physician detailing the method, amount and time schedules by which the medication is to be taken
- A written statement from the parent or guardian of the pupil is on file indicating a desire that the school assists the pupil with medication. This is necessary even for over-the-counter medications such as aspirin, Tylenol, Advil, etc.
- An emergency management plan will be developed by the district nurse for medications such as epi-pens, inhalers, and insulin. Parents must notify the school site of any serious chronic health conditions and potential emergencies such as diabetes, asthma, epilepsy, and life-threatening food or insect allergies.

- Medication Administration forms: for use if your student needs prescription medications and/OR over-the-counter medications (including Tylenol, Advil, etc) to be administered while your child is at school
- Food Allergy & Anaphylaxis Emergency Care Plan (FARE): for use if your student has severe and anaphylactic allergies
- Seizure Action Plan: for use if your student has a seizure disorder
- Asthma Care Plan: for use if your student has asthma and requires an inhaler during the school day. A Medication Administration form will also be accepted.
- Diabetes Medical Management Plan: for use if your student has diabetes
- We prefer to have the DMMP provided by your physician's office but this form is also sufficient
- Release of Information: for use if your student has specific health needs and you would like the school to be able to communicate directly with your student's health care providers (please contact your student's Counselor for this form)
Please submit and return ALL completed forms to your school nurse for review and follow up.
